“How can we treat somebody or care for somebody, without knowing what they want?” Answer: Talk to your patients. On Tuesday, nurses, aides, doctors and other staffers discussed communication problems that plague the health care system during a special conference, called “Mind the Gap.” Gaps occur when patients go from the ICU to rehab or go home after surgery — all without the proper information.
Dr. Jessie Israel of Complete Care organized the gathering in Holmdel to help close patient information gaps with correct medical data and with compassionate care. Speakers focused on promoting accurate medical records, addressing racism in health care, speaking to patients and families about care following discharge and about end-of-life plans.
